Carolyn Diane Richards, 60, of Forest City, died Wednesday, May 18, 2016 at Hospice House. A native of Polk County, she was the daughter of the late Clarence Ed and Elma Katherine Turner Richards.

She is survived by two sons, Sheldon Lewis and Chad Richards, sister, Kay Gee; brothers Phillip Richards and Wayne Richards.

Services will be announced at a later date by the family.
