David Jeffery Forms, 68, died Sunday, August 14 in his home surrounded by his loving family. He was the son of the late David and Betty Forms of Smithers, WV.

His wonderful sense of humor, his political opinions and his love of the Marshall Thundering Herd and the West Virginia Mountaineers will be remembered by his wife of 44 years, Linda Weinberg Forms and his three daughters, Jessica Forms (Baltimore, MD), Amanda (Jason) Clark (Frederick, MD) and Rachel Forms (Shelby, NC) and grandchildren, Tyler, Hunter and Mallory Clark and Adaya and Mikey Howard and his sister, Cathy Hartwell (Richmond, VA), sisters-in-law Karen (Erik) Kanin (Portsmouth, NH) and Bobbie Weinberg (Alexandria, VA) and niece Angela Hartwell Jett (Marshall) and nephew Michael Hartwell (Elizabeth).

David was an Army veteran of the Vietnam War and was employed as a West Virginia coal miner, a nuclear electrician at the Charleston Naval Shipyard and a manager at Lowe's in Shelby, NC. He was a member of New Beginnings Church in Fallston, NC.

A celebration of his life will be held at New Beginnings Church, 4317 Fallston Road, Fallston, NC on Saturday, August 20, 2016 at 2:00pm with Dr. John Ware officiating. Visitation will be one hour prior to service from 1:00-2:00pm in the church sanctuary. Burial will be at Arlington National Cemetery at a future date.
