Maiden - Mr. Jay Phillips, 48, of Country Acres, passed away on Wednesday,
November 1, 2017 at Carolinas Medical Center - Main, in Charlotte.

Jay was born on July 14, 1969 in Johnstown, Pennsylvania, a son of the late
Lenzy Petson Philips and Helen Ruth Vasalinda. He worked in construction.

Survivors include his wife, Diane Hubert Phillips, of the home; two
daughters, Magen Phillips and Sarah Fisher both of Ohio; a son, Nathanael
Naelitz of Georgia; a brother, Lenzy Phillips of Georgia and three
grandchildren, Conner, Sophia and Aiden.

Services will be private.
